Patient Forms
Your initial evaluation will be utilized for exploring your concerns and discussing what services will be useful. A treatment plan will be discussed.
Fees and Payment
The basic fee is $150. Extended individual sessions are available, contact the clinician of your choice for specific quote.
Couples and joint sessions are typically $225 for one and a half hours, $300 for two hours.
Payment is requested at the time services are rendered by cash, check, or credit card.
Insurance
We are a private pay clinic and your insurance benefits would be considered out of network. If you would like to utilize your out of network benefits please contact your insurance carrier for specifics. Believe clinicians will provide a standard HICFA for clients to file out of network benefits after payment.
Cancellations
Cancellations less than 48 hours will be charged full fee.
